Tuesday 8 March 202213:00 – 14:30  CET Unsafe medication practices and medication errors are a leading cause of injury and avoidable harm in health care systems across the world.  WHO Patient Safety Flagship has initiated a series of monthly webinars on the topic of “WHO Global Patient Safety Challenge: Medication Without Harm”,. The main objective of the webinar series is support implementation of this WHO Global Patient Safety Challenge: Medication Without Harm at the country level. Considering the huge burden of medication-related harm, Medication Safety has also been selected as the theme for World Patient Safety Day 2022.Patients and their families are the only constants in the increasingly complex health care systems. A patient may receive care from different facilities and multiple care providers from different disciplines. Every point in the care process contains some inherent risks. Adverse drug events or harm may result from problems in practice, or with products, procedures, or systems, which are not usually the fault of the health care provider. They are a repository of critical information, which they can share when engaged and empowered. Engaging patients and families who have experienced harm can provide insights and learning concerning system failures.
